I acknowledge Lacrosse is a high speed and contact sport, and that practice and games will involve contact. I am aware of no medical conditions or illnesses that could affect my child's performance in practice or game situations. I also hereby give my permission to provide emergency medical assistance to my child in case of injury when I am not present. I also agree to indemnify and hold harmless X-treme Lax Factory LLC, coaches, directors, sponsors, agents or volunteers, for any or all injuries damages, causes of action or civil claims for personal injuries or similar trauma incurred while participating in practice, tournaments or while traveling with the team to tournaments. 2.) By agreeing I/we hereby unconditionally accept the aforementioned terms and conditions under which our child will participate as a member of one of X-treme Lax Factory, LLC girls’ lacrosse teams. 3.) I grant X-Treme Lax Factory, its representatives and employees the right to take photographs of my children. I authorize X-Treme Lax Factory, its assigns and transfers to copyright, use and publish the same in print and/or electronically. I agree that X-Treme Lax Factory may use such photography/videos of my children with or without my name and for any lawful purpose, including for example publicity, illustration, advertising and web content. 4.) WAIVER AND RELEASE: I am fully aware of and appreciate the risks, including the risks of catastrophic injury, paralysis and even death, as well as other damages and losses, associated with participation in a lacrosse event. I agree on behalf of myself, my heirs, and personal representatives, that US Lacrosse, the host organization and the sponsor or sponsors with respect to a Covered Event, together with coaches, officials, volunteers, employees, agents, officers and directors of the host organization and any such sponsors shall not be held liable for any injury, loss of life or other loss or damage as a result of my participation in a Covered Event. This Waiver & Release shall also be for the benefit of and run in favor of any youth organization that requires participants to become members of US Lacrosse as a condition to their participation in such organization's youth lacrosse events, which shall constitute Covered Events for purposes of this Waiver & Release, and any such youth lacrosse league shall constitute the host organization for such Covered Events. 5.) MEDICAL ATTENTION: I hereby give my consent to US Lacrosse and the host organization of and Covered Event to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my participation in Covered Events. 6.) READINESS TO COMPETE: I will only participate in those Covered Events for which I believe I am physically and psychologically prepared to compete. 7.) INFORMATION CERTIFICATION: I certify that all information provided by me in this application, including without limitation my membership category, is true, accurate and complete and I understand that any untrue, inaccurate or incomplete statement or information will automatically invalidate my membership and all the benefits of membership in US Lacrosse. 8.) CODE OF CONDUCT: I agree to all terms (refers to accepted US Lacrosse/Positive Coaching Alliance Code of Conduct). I agree. US LACROSSE CODE OF CONDUCT * Players, coaches, officials, parents and spectators are to conduct themselves in a manner that "Honors the Game" and demonstrates respect to other players, coaches, officials, parents and fans. In becoming a member of the lacrosse community an individual assumes certain obligations and responsibilities to the game of lacrosse and its participants. The essential elements in this "Code of Conduct" are HONESTY and INTEGRITY. Those who conduct themselves in a manner that reflects these elements will bring credit to the sport of lacrosse, themselves, their team and their organization. It is only through such conduct that our sport can continue to earn and maintain a positive image and make its full contribution to amateur sports in the United States and around the world. US Lacrosse supports the following behaviors for those who participate in the sport or are involved in any way with US Lacrosse. The following essential elements of the "Code of Conduct" must be followed: • Sportsmanship and teaching the concepts of fair play are essential to the game and must be taught at all levels and developed both at home and on the field during practices and games. • The value of good sportsmanship, the concepts of fair play, and the skills of the game should always be placed above winning. • The safety and welfare of the players are of primary importance. • Coaches must always be aware of the tremendous influence they have on their players. They are to strive to be positive role models in dealing with young people, as well as adults. • Coaches should always demonstrate positive behaviors and reinforce them to players, parents, officials and spectators alike. Players should be specifically encouraged and positively reinforced by coaches to demonstrate respect for teammates, opponents, officials and spectators. • Players should always demonstrate positive behavior and respect toward teammates, opponents, coaches, officials, parents and spectators. • Coaches, players, parents and spectators are expected to demonstrate the utmost respect for officials and reinforce that respect to players/teammates. Coaches are also expected to educate their players as to the important role of lacrosse officials and reinforce the ideal of respect for the official to players/ teammates. • Grievances or misunderstandings between coaches, officials or any other parties involved with the sport should be communicated through the proper channels and procedures, never on or about the field of play in view of spectators or participants. • Officials are professionals and are therefore expected to conduct themselves as such and in a manner that demonstrates total impartiality, courtesy and fairness to all parties. • Spectators involved with the game must never permit anyone to openly or maliciously criticize badger, harass or threaten an official, coach, player or opponent. • Coaches must be able to demonstrate a solid knowledge of the rules of lacrosse, and should adhere to the rules in both the letter and the spirit of the game. • Coaches should provide a basic knowledge of the rules to both players and spectators within his/her program. Attempts to manipulate rules in an effort to take unfair advantage of an opponent, or to teach deliberate unsportsmanlike conduct, is considered unacceptable conduct. • Eligibility requirements, at all levels of the game, must be followed. Rules and requirements such as age, previous level of participation, team transfers, etc, have been established to encourage and maximize participation, fair play and to promote safety. For any participant who is not yet 18 years old: As legal parent or guardian of this participant, I hereby verify by agreeing below that I fully understand and accept each of the above conditions for permitting my child to participate in a US Lacrosse sanctioned event and accept each of the above conditions, and especially the waiver and release set forth in paragraph one. At Xtreme Lacrosse, we fully support and abide by the US Lacrosse Code of Conduct and wish to further reinforce the importance of sideline behavior from our parent community and spectators. Parents and spectators play a critical role in creating a positive environment for our athletes. You are expected to support not only your own child, but the entire team and program. Cheering and encouragement are welcomed and appreciated, but any negative comments, yelling, or criticism directed toward players (on either team), coaches, officials, or fellow parents will not be tolerated under any circumstances. This type of behavior is damaging to the culture we are building and undermines the lessons of sportsmanship and respect we strive to teach. Any parent or spectator who engages in negative or disruptive behavior may be subject to disciplinary action, including removal from the field and/or dismissal from the program without refund. We take this responsibility seriously and ask that all families model the highest level of sportsmanship on and off the field. Player Expectations * Be Punctual...EARLY is on time, on time is LATE. • Be at practice 15 minutes early, and be dressed--shoes tied, goggles on--ready to start on time. • Tournaments: Be at your designated field 45 minutes prior to game time ready to warm-up. Commit to Attend • Attendance at practice is MANDATORY. Practice is the time to improve your game, and the time in which your team will learn to work together. Missing practices and/or tournaments will impact playing time, and if excessive, removal from the team. • Tournaments are MANDATORY unless a prior agreement has been made. • Understand that you are a member of a TEAM and the TEAM comes first. • Playing time is earned and NOT all players will earn the same playing time. • The coaching staff reserves the right to play each player in the position that helps the team be successful. A player may prefer to play midfield; however, the coaching staff believes it better serves the team if they are on attack. Doing what is best for the team is a critical component to the success of any program. • Any player who plays for another club as a “guest player”, in tournaments or training sessions, will be removed from our program with no REFUND. Attire • Practices: X-treme Lax Pinnies or X-treme Lax t-shirts, and all proper equipment worn (goggles, mouthguards, etc.) • Tournaments: Jersey and uniform shorts, and all proper equipment worn (goggles, mouthguards, etc.) Sportsmanship first and foremost! • Respect your coaches • Respect your teammates • Respect (and appreciate) your parents • Respect your opponent • Respect officials • Respect the game Communication • Advocate for yourself. YOU should be the one talking to your coaches, NOT your parents. • Listen well to your coaches and teammates. Playing Time Policy • X-treme Lax Factory teams are competitive teams and their objectives are to perform at the highest level. Playing time must be earned. Coaches may start, and may play more, the girls that provide the best opportunity for success on the field. Factors such as missed practices, missed tournaments, effort and attitude may impact a player’s playing time. X-treme Lax Factory reserves the right to add or remove players from rosters at any time. All players are expected to work hard and demonstrate a positive attitude and willingness to improve and develop their skills on an ongoing basis. All players with good attendance and attitude will receive significant but not necessarily even playing time. There is no minimum playing time requirement. Equal playing time will be more of the focus in our youngest teams. For high school teams, understanding the goal of attending true recruiting tournaments, the coaches will attempt to give all players significant amounts of game time. Be a good teammate • Support your teammates on and off the field. • Come ready to play and willing to learn. • The only things you have complete control of are your ENERGY LEVEL, EFFORT and ATTITUDE. Give 100% of all three, each and every time you step on the field. Agree. Team Fee/Credits/Cancellations/Refunds * The team fee can be paid in two installments: The down payment is due by July 28, and acts as your commitment to play with X-treme Lax Elite. This payment is used immediately to purchase uniforms and register for tournaments. The remaining balance is due by January 12. The fees also cover: • Quality coaching to improve individual and team skills • Field rentals and insurance fees • Use of coaching staff for college references • Team Uniform (fall tournament & summer teams receive uniforms) • Once a player has registered for X-treme Lax Elite, the payment is non-refundable. • A player WILL NOT be able to participate in games/tournaments until payment is received. If a player has an issue with the payment schedule, please communicate with X-treme staff to work out an acceptable arrangement. Weather-Related Cancellations: In the event of inclement weather, safety will always be our top priority. While we make every effort to hold practices as scheduled, there may be instances when sessions must be canceled due to weather conditions. Please note that practices canceled due to weather will generally not be rescheduled or made up, except in rare cases and at the sole discretion of the club. Missed Practices: If a player misses a scheduled practice for any reason (including illness, vacation, injury, or other personal conflicts), that session will not be made up and no refund or credit will be provided.
